Prior to the course, I completed the required intake documentation and clearly disclosed a pre-existing medical condition involving persistent migraines following a head injury. I have copies of this documentation, signed and acknowledged by Matthew Clemente, the PADI-certified instructor for my course. Despite this disclosure, I was permitted to continue participating in the course on June 25, 2023.
After I withdrew, I was contacted by Jemma Purton, also a PADI-certified instructor for the course, and asked to obtain a doctor’s sign-off after the fact for my participation. My physician refused and advised that allowing me to dive under those conditions could have posed a risk of serious harm, including permanent injury or death.
Following a billing dispute, my elderly parents in Canada were subjected to unannounced home visits by Peter Mikhail, owner of Toronto Scuba Centre, accompanied by Daina Slenys, who represented herself as acting in a legal capacity. These visits occurred despite both individuals being informed that I do not reside at my parents’ address and live outside the country.
I found this conduct wholly inappropriate and distressing for my family. A business operating in a safety-critical field should demonstrate sound judgment, strict adherence to its own medical screening procedures, and professionalism when disputes arise.
Despite again formally notifying Peter Mikhail and Daina Slenys in writing on January 17, 2026 of my correct mailing address, email address, and phone number, Mr. Mikhail again attended my parents’ home approximately two weeks later and harassed my elderly mother.
Based on my experience, prospective students should exercise caution and consider alternative training providers.
